Applicants Age 15 or Under Without Both Parents Present
The DS-3053 is the Statement of Consent for Issuing a Passport to a Child and the DS-5525 is the Statement of Special Family Circumstances.
Adult Passport Renewal (DS-82)
Renew an existing passport issued to you when you were aged 16 or older.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
Report a passport lost or stolen and replace it.
You can fill out the correct form, or find information about your passport status at https://travel.state.gov.
You must provide one photo on a white background with your passport application. Our photo policies apply to adults and children. We have a few exceptions to our photo policies for infants.
If you are travelling abroad and your passport validity expires, you can apply for a full-validity regular passport or a limited-validity emergency passport. Replacing a full-validity passport requires a minimum of two weeks. An emergency passport can usually be issued within two business days.
