Event: New Evacuation Questionnaire
With the Government of Nigeria’s extension of airport closures to international flights, U.S. Mission Nigeria is gauging the interest of U.S. citizens in an evacuation flight to the United States. To determine interest, we have developed a new evacuation questionnaire to collect the most accurate information from U.S. citizens. To be counted accurately, everyone with serious interest in evacuating MUST complete the below questionnaire, even if you have already completed previous forms.
If you are seeking to return to the United States on an evacuation flight, please CAREFULLY and ACCURATELY complete the following questionnaire for the location nearest to you:

EACH traveler in your party must complete his/her own questionnaire in order to be included in our data. Please use only these forms to express interest in an evacuation flight. All other forms have been disabled.
Please note: U.S. Mission Nigeria does not have any evacuation flights scheduled at this time. We will continue to monitor the situation in Nigeria closely and determine the need for additional evacuation flights. Please continue to monitor your email and phone for further communication on the availability of future flights.
